Copies of birth and death certificates for natives of the city of Hamilton are available through the Health Department.

Birth and death certificates may be requested by telephone, mail (please include a self-addressed stamped envelope) or in person for a fee of $20.00.  Cash, personal check, money orders or Visa and MasterCard are acceptable forms of payment.

Please provide the following information when requesting a birth certificate:

Date of Birth
Full Name
Father's Name
Mother's Name, including maiden name
Telephone number

Please provide the following information when requesting a death certificate:

Deceased's Full Name
Date of Death
Place of Death (i.e. Hospital, residence)
